In providing services for clinics, Professional Dosimetry Services (PDS), uses certain protected health information (PHI), including but not limited to, electronic protected health information (EPHI). PDS will make every effort to comply with the provisions in HIPAA requiring a "Business Associate" to provide adequate assurances to a clinic with respect to the security and confidentiality of PHI.
In the course of performing its duties and obligations, PDS may receive, create and/or use certain health-related information concerning clinic patients that identifies, or can reasonably be linked to the identity of such patients. To the extent that this information constitutes PHI as defined by HIPAA, PDS shall consider this information to be PHI.
PDS shall use appropriate internal safeguards to prevent the use or disclosure of PHI and EPHI. PDS shall also retain administrative, physical and technical safeguards that appropriately protect the confidentiality, integrity and availability of PHI and EPHI that PDS creates receives, maintains or transmits on behalf of clinics.
